Follow up appointment
If you are being discharged from our inpatient ward, we will give you details of your 72 hour follow up appointment.
This will include:
- Ward name
- Your care co-ordinator / lead professional
- Their contact details
- Date and time
- Your GP’s contact details
Contact details post discharge from our inpatient ward
Between 9am to 5pm, Monday to Friday:
You can contact your care co-ordinator / lead professional or the duty worker.
At any other time:

Medication
We will give you an agreed supply of medication. Please make an appointment with your GP to arrange a further prescription. Your GP’s number is listed above. Find out more about your medication at www.tewv.nhs.uk or speak to your GP.
